If you think any level of pain relief provides a pain free state at those altitudes then you have been blessed enough never to have witnessed it.

The people tending him and exposed to him should be spared witnessing abject agony on a daily basis. Unless you've a pretty significant personality disorder, that kinda thing has a way of getting to you.

I don't think you realise how much of a dickhead Milat was in his life. If he had not a psychopathic personality, he would not have killed a single person. And if I had sociopathic tendencies, I would not wish painkillers be withheld from Milat during his cancer. Being revenge-minded is not a unique trait of sociopathy. Otherwise, we would have a heap of sociopaths around.
